Who May Qualify?
To receive services through ALTCS, an individual must:
- Meet medical eligibility requirements.
- Meet Medicaid financial eligibility guidelines.
- Need assistance with daily activities such as bathing, dressing, eating, mobility, or supervision.
Step 1: Apply for ALTCS
The first step is determining whether your loved one qualifies for ALTCS services.

Completing an application through AHCCCS online
Step 2: Determine the Appropriate Service Program
Once approved, individuals are placed into a program that best meets their needs. Programs may include:
ALTCS Elderly and Physical Disability (EPD) Program
For older adults and individuals with physical disabilities who require long-term care services.
Division of Developmental Disabilities (DDD)
For qualifying individuals with developmental disabilities who need long-term support services.
Step 3: Choose How Care Is Provided
Care services may be delivered through:
Agency-Based Care
- A contracted home care agency is assigned.
- The agency hires and manages caregivers.
- The agency handles payroll and employment requirements.
Self-Directed Care Options
Some programs allow participants greater flexibility in choosing caregivers, including certain family members, depending on program rules and approval.
Important: Caregivers are not paid directly by Medicaid. Payment is administered through an approved agency or financial management system.
Step 4: Complete Caregiver Requirements
Once selected as a caregiver, individuals may need to complete:
- Background checks
- Fingerprint clearance (when required)
- State-required caregiver certifications
- Agency onboarding requirements
- Initial training and evaluations
Training and onboarding requirements vary depending on the program and agency.
Beginning Care Services
After approval:
- A care plan is developed outlining approved services and hours.
- Caregivers provide services according to the care plan.
- Timesheets or electronic visit verification may be required for payment.
